INTotal Health offers free Case Management help to our members. Case Management is a program that can help you coordinate your care or help you find resources if you are facing a major or complex illness, have many care needs and healthcare doctors, or are at a high risk for issues because of your health condition. Some common issues that often benefit from a Case Manager’s support are High-risk pregnancy, mental health concerns, cancer, premature babies, ill children, members with multiple healthcare needs or members who have lot of illnesses or injuries. A skilled nurse or healthcare staff member works with you to find the problems and works to solve them. You can self-refer to this program. INTotal Health offers a free Disease Management program to members. Disease Management is a program that helps members with certain chronic medical condition better manage their own health using support from our own skilled nurses and other medical staff. A chronic condition is an illness that may never go away, require ongoing medical care, and are sometimes difficult for members to manage without support. INTotal Health offers Disease Management for the following medical states: Diabetes, Asthma, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD or Emphysema),and Coronary Artery Disease (CAD).
Our skilled nurses and healthcare staff can give you resources and teach you how to keep yourself well; whether this is a new medical condition or one you have had for a while.
